Submit a Concern or Complaint

Concerns or complaints are best handled locally with your Sahtu Health and Social Services practitioner. Your practitioner will be able to resolve an issue in a timely manner; if they cannot, they know who to contact to get it resolved. In the rare situation that an issue cannot be resolved locally please fill out the Client Concern and Consent forms and fax to: Attention CEO, 867-587-3436.

Concerns directed to the MLA or Minister of Health and Social Services are referred to the CEO to resolve resulting in less timely resolution due to the additional tracking required at multiple levels of government.
